Add section on HDR to HDR tone-mapping

The strawman should explicitly mention HDR to HDR tone-mapping, as opposed to HDR to SDR tone-mapping. In particular, Annex 5 of Rep. ITU-R BT.2408-5 is also intended to work for HDR -> HDR tone-mapping.
