[mdspan.sub.helpers] doesn't say the exposition-only helpers are exposition-only

Similarly for [mdspan.extents.expo] and maybe others. The title of the subclause is "Exposition-only helpers" but I don't think titles are normative, so we should start the subclause with something like "the following exposition-only funtions are used ..."
