setup() and output() silently fail

Wasn't running as root, couldn't figure out why my GPIO.output(...) calls didn't do anything. Looked in the library code, which doesn't seem to check for errors. That's bad.

There were several improvements for error handling in #108. Could you please check if this is still an issue?
