An error occurred processing the current state

I'm running on a custom branch based on current main and swapneils/main which has a patch to fix the tool trusting flag for mcp servers.

I was about 30 prompts (or more?) into a conversation and started receiving the following error after every prompt even a simple "what is the date?"
